The success of deep brain stimulation (DBS) for movement disorders and the improved understanding of the neurobiologic and neuroanatomic bases of psychiatric diseases have led to proposals to expand current DBS applications. Recent preclinical and clinical work with Alzheimer's disease and obsessive-compulsive disorder, for example, supports the safety of stimulating regions in the hypothalamus and nucleus accumbens in humans. These regions are known to be involved in addiction and overeating associated with obesity. However, the use of DBS targeting these areas (...) as a treatment modality raises common ethical considerations, which include informed consent, coercion, enhancement, threat to personhood, and manipulation of the reward center. Pilot studies for both of these conditions are currently investigational. If these studies show promise, then there is a need to address the ethical concerns related to the initiation of clinical trials including the reliability of preclinical evidence, patient selection, study design, compensation for participation and injury, cost-effectiveness, and the need for long-term follow-up. Multidisciplinary teams are necessary for the ethical execution of such studies. In addition to establishing safety and efficacy, the consideration of these ethical issues is vital to the adoption of DBS as a treatment for these conditions. We offer suggestions about the pursuit of future clinical trials of DBS for the treatment of addiction and overeating associated with obesity and provide a framework for addressing ethical concerns related to treatment. Two overriding considerations shaped the development of early research on the biological effects of microwave radiation—possible medical application and uncertainty about the hazards of exposure to radar. Reports in the late 1940s and early 1950s of hazards resulting from microwave exposure led to the near abandonment of medical research related to microwave diathermy at the same time that military and industrial concern over hazards grew, culminating in the massive research effort known as ‘the Tri-Service program’ . The constraint formalism of classical mechanics is extended to field theories with gauge groups. Explicit examples of Klein-Gordon and Maxwell fields are presented. The symmetry properties of the Maxwell fields have the unexpcted feature in this formalism of forming a first-class algebra which is not Lie, a situation already encountered in the general theory of relativity.

Live donors comprised approximately 5,769 (34%) and 247 (4%) of all kidney and liver transplants in 2011, respectively (OPTN/UNOS). The reasons why people donate, the perception that donating (...) does not entail a decision per se, and the attendant fears and anxieties living donors feel about donating, have all been previously well investigated (Lennerling, Forsberg, Meyer, & Nyberg, 2004; Papachristou, Walter, Frommer, & Klapp, 2010). But much less is known about how donors experienced the overall donation process including the donor evaluation, preparation, appreciation of risks, decision-making, informed consent, post-operative, recovery processes, and the impact of donating on all aspects of their life, from their point of view. Thus, a narrative approach is essential for gaining insights into these less well known phenomena.There is good reason to remain concerned about the safety and ethical basis of living donation considering that living donors undergo serious health risks for no direct medical benefit to themselves (Ethics Committee of the Transplantation Society, 2004; Laird, 2012; Neergaard, 2011; Ratner & Sandoval, 2010; Shelton, 2011). The situation raises serious ethical questions specifically about nonmaleficence and informed consent, and more broadly about using people as a means to an end. The absence of long-term information may not undermine a willing donor’s commitment to save a loved one’s life, but is certainly cause for concern among transplant clinicians who want to be sure to disclose information that may be material for donors in order to provide adequate informed consent. In other words, the disjuncture between what transplant clinicians find important to disclose to donors and what prospective donors need to know—which follows along the lines of what Elliot Mishler referred to as the ‘voice of medicine’ versus the ‘voice of the lifeworld’(1984)—seems to complicate donors’ preparation for donation. Donors’ stories are important for addressing these concerns by identifying risks that typically do not get reported in the scientific literature—perhaps owing to a different [End Page 1] conceptualization of what should be construed as noteworthy risks—as well as by demonstrating how personal situations can enhance or detract from the experience overall.This issue of Narrative Inquiry in Bioethics (NIB) affords a unique entrée into both the often heard and unheard stories of living organ donors. A set of topical areas were presented for the authors’ consideration that relate to the donation process in general and the informed consent process more specifically: whether donors were prepared to donate; comprehended what they were told; could have understood the process better, as well as, the impact of donation on health, employment, finances, and personal relationships (Table 1). When soliciting donors’ stories, every effort was made to include a full range of experiences—both good and bad—from a variety of donors: kidney donation, liver donation, adult-to-adult and adult-to-child donation, related, unrelated, directed and anonymous (also known as ‘altruistic’ or ‘good Samaritan’ donors), paired chain exchange, and list exchange (Table 2)—in order to present a balanced perspective on live donation. Additionally, this issue includes a full breadth of stories about the donor experience, ranging from positive to mixed to negative (Table 3). While donors were given tremendous leeway in what stories they told, they were asked to focus their narratives on the lesser known dimensions of donation—the experience of it. This paper first elaborates ‘Art’ and the aesthetic as these concepts emerged in the eighteenth century, and uncovers the conflict between the resulting ideal of ‘art for art’s sake’ and the increasing use ‘art therapy’ for personal and social purposes. Taking this conflict to be a reason for the rejection of ‘Art’, it considers two accounts of ‘the end of Art’, one by Arthur Danto and the other by Nicholas Wolterstorff. The paper argues that both accounts fall short of (...) adequately recognizing the presence of the aesthetic in everyday life, and that once we do acknowledge the properly aesthetic character of ‘Design’, a way of thinking is opened up that can make better sense of art therapy. Rosenblatt; Princeton: Princeton University Press, 1994, $39.50.The epic project that includes the poems Paradise Lost and Paradise Regained marks the last occasion in Europe when the most ambitious literary form sought stability in theology rather than in philosophy. The philosophical poem, a minor form before the Enlightenment, became after Milton the general (...) idea of what a great long poem should be. One thinks of Faust, naturally enough, where the triumph of philosophy over theology is enacted, of Coleridge’s youthful plans for a philosophical epic, and of Wordsworth’s qualified achievement of it in The Prelude. Hegel said that art, having been supplanted by philosophy as the discourse of truth, must be for us a thing of the past. A less striking but perhaps more accurate statement would be that the art that once sought stability in the knowledge of God now seeks it in the impersonal knowledge of truth. The Milton who, before Paradise Lost, composed for his own use a theological treatise in which every point made is anchored in scripture, belongs to an earlier age. Yet there are in him clear indications of what is to come. No poet before Milton is so preoccupied with metaphysical questions, with the ultimate nature of being and the causative principles governing creation.It is the theological Milton with which Jason P. Rosenblatt is concerned in this fine study of Paradise Lost, a study that is more controversial than is suggested by its title or by its eirenic scholarly tone. The book is an attack on what its author regards as the Pauline bias of Milton studies in the tradition of A. S. P. Woodhouse and Arthur Barker. It is a bias, persisting even in Miltonists who are unconcerned with theological questions, against Hebraism in general and the “Old Law” in particular, the system of law referred to as Torah. Expressed in positive terms, Rosenblatt’s aim is to show how Milton’s prose tracts of 1643–1645 (the tracts on divorce) are “the principal doctrinal matrix of the middle books of Paradise Lost” (p. ix), that is, of the books in which human beings are shown in the state of innocence, giving us a utopian model [End Page 546] of what we should strive for in our own lives. But the Hebraic ethos of Eden gives way in the later books of the epic, after the Fall, to a Pauline perspective where the Law is an evil that stands in the way of our freedom. Whereas Pauline theology condemns the Hebrew Law as a law of bondage, Torah, like the Tao, is a means to live freely in accord with the law of one’s nature. Such law is felt not as a restraint but as a path. Rosenblatt suggests that Paradise Lost is not the unified, orthodox Christian vision Woodhouse claimed it to be but a work that undergoes radical ideological change after the Fall, reenacting the historical change brought about by Christianity itself.In the 1640s Milton embarked on an arduous program of Hebraic studies, spurred on by his desire to recuperate the one Mosaic law Christ explicitly overturned: that permitting divorce for causes other than fornication, a word that, as Milton’s Satan says of another, bears no single sense. After the exaggerated assessments, earlier this century, of Milton’s Hebrew erudition an over-correction set in, asserting that because Milton resorted to secondary sources, reference works, and translations his Hebrew learning was shallow. But Milton never pretended to be a Hebrew or an Aramaic scholar. He knew the languages and used them to learn more, as he used and acknowledged the work of great scholars like Selden; and for an amateur his attainments were truly remarkable. Miltonists will value this book for its balanced, expert reassessment of the poet’s Hebrew learning. Genocide in Rwanda, multiple murder at Denver or Dunblane, the gruesome activities of serial killers - what makes these great evils, and why do they occur? In addressing such questions this book, unusually, interconnects contemporary moral philosophy with recent work in New Testament scholarship. The conclusions to emerge are surprising. Gordon Graham argues that the inability of modernist thought to account satisfactorily for evil and its occurrence should not lead us to embrace an eclectic postmodernism, but to take seriously (...) some unfashionable pre-modern conceptions - Satan, demonic possession, spiritual powers, cosmic battles. One of the most basic questions an ontology can address is: How many things, or substances, are there? A monist will say, ‘just one’. But there are different stripes of monism, and where the borders between these different views lie rests on the question, ‘To what does this “oneness” apply?’ Some monists apply ‘oneness’ to existence. Others apply ‘oneness’ to types. Determining whether a philosopher is a monist and deciphering what this is supposed to mean is no easy task, especially (...) when it comes to those writing in the early modern period because many philosophers of the sixteenth, seventeenth, and eighteenth centuries include God in their ontologies. InThe Principles,Anne Finch Conway offers an ontology that is often described as being both ‘vitalist’ and ‘monist’. I take this to mean that, for Conway, all that exists is in some way alive and that if asked ‘How many things, or substances, are there?’ Conway would say, ‘Just one’. But to what does this ‘oneness’ apply? And where does the point of disagreement between Conway and her interlocutors, Hobbes, Spinoza, More, and Descartes lie? In this paper, I argue that determining the answer to this first question turns out to be quite difficult. Nevertheless, we can still make sense of the second. (shrink)

Contemporary health care often lacks generosity of spirit, even when treatment is most efficient. Too many patients are left unhappy with how they are treated, and too many medical professionals feel estranged from the calling that drew them to medicine. Arthur W. Frank tells the stories of ill people, doctors, and nurses who are restoring generosity to medicine--generosity toward others and to themselves. The Renewal of Generosity evokes medicine as the face-to-face encounter that comes before and after diagnostics, pharmaceuticals, (...) and surgeries. Frank calls upon the Roman emperor Marcus Aurelius, philosopher Emmanuel Levinas, and literary critic Mikhail Bakhtin to reflect on stories of ill people, doctors, and nurses who transform demoralized medicine into caring relationships. He presents their stories as a source of consolation for both ill and professional alike and as an impetus to changing medical systems. Frank shows how generosity is being renewed through dialogue that is more than the exchange of information. Dialogue is an ethic and an ideal for people on both sides of the medical encounter who want to offer more to those they meet and who want their own lives enriched in the process. In order to make sense of Scotus’s claim that rationality is perfected only by the will, a Scotistic doctrine of truth is developed in a speculative way. It is claimed that synthetic a priori truths are truths of the will, which are existential truths. In this paper we show how a realistic normative democratic theory can work within the constraints set by the most pessimistic empirical results about voting behaviour and elite capture of the policy process. After setting out the empirical evidence and discussing some extant responses by political theorists, we argue that the evidence produces a two-pronged challenge for democracy: an epistemic challenge concerning the quality and focus of decision-making and an oligarchic challenge concerning power concentration. To address the challenges we then (...) put forward three main normative claims, each of which is compatible with the evidence. We start with a critique of the epistocratic position commonly thought to be supported by the evidence. We then introduce a qualified critique of referenda and other forms of plebiscite, and an outline of a tribune-based system of popular control over oligarchic influence on the policy process. Our discussion points towards a renewal of democracy in a plebeian but not plebiscitarian direction: Attention to the relative power of social classes matters more than formal dispersal of power through voting. We close with some methodological reflections about the compatibility between our normative claims and the realist program in political philosophy. (shrink)

I argue that both are true, Fulfilling the requirements of scientific psychology and the phenomenology of mind, Respectively. The key to understanding their mutual consistency lies in the idea of parallelism and its corresponding requirement that all descriptive features of mental states be analyzed as properties, None as relations (to anything physical). So the intentional link itself must be a 'logical' and not a descriptive (...) connection. More broadly, It is required to free the act from its origins in the metaphysical notion of 'activity' and the substance ontology from which that motion derives. (shrink)

ABSTRACT This essay is a critical study of Jason Brennan's Against Democracy. We make three main points. First, we argue that Brennan's proposal of a right to competent government only works if one considers the absence of government a viable proposition, something most of his opponents are not prepared to do. Second, we suggest that Brennan's account of competent decision-making is blind to forms of oligarchic power that work against the very ideals of justice and epistemic virtue that competence is (...) meant to safeguard. Third, we muster empirical evidence to argue that, in the real world, democracy is not just about making decisions and selecting policies, in which case Brennan's argument misses its mark. (shrink)

According to the theory of dispositions here defended, to have a disposition is to have some (non-dispositional) property that enters into a law of a certain form. The theory does not have the crucial difficulty of the singular material implication account of dispositions, but at the same time avoids the unfortunate notion of 'reduction sentences'. It is further argued that no dispositional explanation is one of the covering-law type; but the theory shows how, for any dispositional explanation! To construct a (...) potential explanation of the covering-law type. The theory can also be applied fruitfully to human behavior, especially with respect to the issues of reasons and causes and of' rational' explanation.
